Go to your App Store or Google Play and download the "Concussion Tracker" App
Step 2
Create New Account
Please click create a new account to get started. (If you already have an account set up you can sign in here)
Step 3
Complete your Neurocognitive and Balance Tests
Please complete both the Neurocognitive and Balance tests.
In-Person Concussion Testing
In-person testing is not required for a complete pre-season baseline but can add additional information post-concussion. If you'd like additional testing please schedule an appointment by calling our office.
